Massively enlarged spleen

Lymphoma

A 20 year old man presented with acute abdominal symptoms and was found to have a greatly enlarged spleen. [intact spleen.]

The resected spleen is massively enlarged, and a tumor mass is visible through the capsule. [intact spleen.]

The 1320 gram (nearly 3 pound) spleen was 230 mm long, and the tumor within it measured 180 mm (7 inches) in greatest dimension. [slab of spleen.]

A slice through the spleen shows a large well demarcated tumor which on frozen section appeared to be a lymphoma. (Flow studies at Vanderbilt showed a monotypic B cell large lymphocyte population that expressed monotypic kappa and coexpresses CD10, compatible with a follicular origin.) Large areas of necrosis are present in the tumor. Adjacent apparently normal spleen is visible on the viewer's left. [ct scan of spleen.]

Spiral CT scan shows a 7 mm cut, with the greatly enlarged spleen (on the viewer's right), showing the less radiodense tumor mass with the somewhat denser adjacent normal spleen. IVP contrast medium provides enhancement.
